Start new job tomorrow, got signed off by doctor for a month on Friday, advice needed

I am terribly sorry if this is the wrong place, I really needed some advice. I am 21 years old, female and have Asperger's, Asthma, IBS and Dyspraxia, I have been signing on JSA for the past six months since having to drop out of college prematurely (managed to get a good grade in my first year of level 3 health and social care, but couldn't afford to progress on the second year as travel to college was too much)
(I know that being on benefits is considered taboo for some, but my parents need the money for rent, bills etc) And have been desperately seeking a job, I finally had an offer for an apprenticeship, due to start tomorrow. However, things finally came to a head this week, feel like I can barely breathe so went to the doctors on Friday, as have been stressed out for months and feeling extremely anxious, also since I started signing on I have been getting one bout of tonsillitis after the other as well as ear infections, finally have an appointment with the ENT department to put me on the waiting list to have them removed.
The doctor concluded I am suffering from a chest and sinus infection as well as an ear infection and tonsillitis, things spiked as of Wednesday/Thursday which led me to making the appointment, I asked him if there was anyway I could be able to have a long term prescription for antibiotics (lower dose) and ear drops.
He then conducted an examination of my neck and found several lumps still there... so he's signed me off for a month, also placed me on anxiety medication (which is helping) and more pain meds, antibiotics and also ear drops and nasal spray but wants me to sign off for a month, has given me a sick note... (though hasn't actually ticked that I am not fit for work but has said I am now signed off for a month to be reviewed at the end of the month to allow my meds to kick in and to allow any tests and ops to be carried out if needed)
What worries me is I said to the employer on Tuesday I would be fine to start for this coming Monday (tomorrow) as thought I would be... I had also yet to sign off JSA as lost my voice on Friday (ended up typing up what I needed to ask the doctor), all he said to me was to take the note to the Job Center...
What I was wondering though, is what do I say to the job center? do I just take the note in and say "the doctor told me to give this to you?" as far as the employer goes, I have emailed her a very apologetic email and a scanned copy of the note, but was wondering do I need to do anything else? I know they won't want to wait for me for a month (the employer is a nursery)... as they needed someone to start this week, am gutted as really wanted to start with them, but honestly, feel like I have been hit by a bus. I have a long term plan on where I can go to gain employment after my tests are completed or for work experience, as need my confidence back.

Tomorrow was meant to be I think a official signing of the contract *as have yet to on paper* and induction, thanks in advance to anyone who can help... just feel so worried about it all

Try not to worry too much, easier said than done I know.
Take your doctors note to Job centre tomorrow and ask for their advice
If you are in doubt make an appointment at the citizens advice they are helpful too.
Not really sure exactly what you are asking but as you can't start work because you are unwell you won't be able to sign of job seekers, also you will need to wait and see what your new employer says, they may still want you but until you hear from them you won't knowThe mind is like a parachute. my voice (after much gargling with honey/lemon) has started coming back today... I have emailed her, I guess waiting is all I can do, I was just kinda wondering if anyone had been in a situation like this... I shall take it into the job center and see what happens (sorry for how odd my post seemed, I suck at explaining things o.O ) thanks again

I've started a job sick and honestly not sure I recommend, that feeling of your mind going blank, unable to write, god it wasn't nice and took ages to get past, take care it took a couple of weeks to feel right again0 -

From 30/03/15 if a JSA participant suffers a third or a longer period of sickness, they can volunteer to stay on JSA for up to 13 weeks in a rolling 12 month period on an Extended Period of Sickness (EPS) rather than claim ESA. The EPS is one continuous period of 13 weeks or less and cannot be split into separate periods. The 12 month rolling period is calculated from the first date of the EPS.
The choice to take an EPS is voluntary, but if they do decide to stay on JSA, participants are expected to remain close to the labour market, with personalised conditionality and support tailored to their health condition or illness. The Jobcentre Plus (JCP) Work Coach will agree with them their temporary conditionality.
An EPS can last for up to 13 weeks and the participant must provide supporting medical evidence, for example, a Statement of Fitness for Work (known as a Fit Note) from their GP.0 -
