Duplicated Defaults On Credit Files

First post hope it is in the right forum.

Having checked my credit file i find that Barclaycard and Capital One have both defaulted me. Also the DCA's involved have also both defaulted me hence i have duplicated entries.

I understand that i can only be defaulted once for each account. I have sent CCA requests to both DCA who have now committed criminal offences by not providing the information.

Has anyone any suggestions for a letter to send in order to get the dupicated entries removed. If possible by Barclaycard and Capital One. Then I will repeat the process with the DCA in the hopes of removing the entries altogether
