Why did I get a self assesment tax form when I've been employed for over a year?

If it's not an error but has been received very recently it may have an extended deadline. It will say on the notice.

Does anyone know why have I been sent a self assesment tax form when I have been employed by the council since October 2013?
I was previously on ESA for approx 2 years.

I got forced to do self assessment because there was a small amount of tax underpaid from a previous year that for some reason they didn't add to my tax code (as they had done previously).
Another common reason is receiving child benefit and earning more than £50k pa.0
