Looking for a template

Greetings, I am in a shared ownership situation and can no longer afford to sustain it and am looking for a template, of sorts, on how to approach my Mortgage provider to address this situation. I am in negative equity and can't continue this way, as acquiring more shares in literally impossible.
Any advice given would be exponentially appreciated.
