Wem

Hi y'all.
I just got an email from a company called "inmoment" and "Mitchell and butlers" saying I've win £1000. Obviously I'm over the moon, but at the same time weary for obvious reasons. However; there are reason to be less weary than others but I'd still like advice.
A) is addressed to me. I.e. It says Dear Tom
B) it's actually from a customer service survey
C) Mitchell and butlers is harvester which I remember doing a survey from, and
D) they've not asked for bank details etc, just a signature, my address and a witness name and signature.

Should I fill the form out and send it before the 28th which is the cut off, or avoid?
Thanks
