when I am listing creditors on my forms - if the debt has been sold on to a collection agency, do I list the agency or the original bank?

I listed the company who owned the debt at the time of going bankrupt.0

Agreed - list the most recent owner of the debt, randomrules.
The main purpose of doing this is to allow creditors to be made aware of the bankruptcy.

Both are needed.Where the debt has been purchased or taken over by another firm provide the name of the original supplier, e.g. Barclaycard, but the name and address of the company which has taken over the debt collection.
i.e. in the fashion of the last item for ZT Telephones in the example in this link.
