Holiday Request Form??

Hello everyone. As title says i have an issue with my holiday request form. I am working in a hotel . Usually working days for me are wednesday-friday-saturday-sunday. The hours it depends how busy the hotel is but usually i work 4-6 hours. Now i said to my manager that i need to take 2 weeks holidays and she asked me to fill a holiday request form. On the form i need to write how many days i will take when i am going to return at work and what days from the days i ll take are unpaid,day off,holiday etc. Now my question is: Should i write requested days all the 15 days i am going to leave or just the working days i used to work.? For example i wrote: From the 15 days i will leave i request 9 days holidays the rest days are day off. Is this right?Also the 5 of 9 days i wrote as holidays are weekends, because i work weekend. Do they pay weekends as paid holidays?Thank you !:cool:

(The holiday you are requesting would appear to be 2 weeks of 4 days, so you want 8 days, don't you?) Perhaps this needs clarification around what they mean by a "day off".) Is this right? Also the 5 of 9 days i wrote as holidays are weekends, because i work weekend. Do they pay weekends as paid holidays? (Weekend days, when you normally work them, are no different from weekdays. They would be paid at your normal rate.) Thank you !:cool:
What you also need to establish is how they will calculate a day's pay for you. If you have a low hours contract then they would use the low hours (rather than your regularly worked hours) to work out a day's rate. You need to look at your written particulars which may include a staff handbook to explain some of the detail.0 -
I am in zero hours contract and the contarct says tha the part time workers will get paid pro rata holidays. In terms of the form should i write as holidays only the days i used to work(8 days)? And i should write the rest days(7 days) as unpaid holidays? The choices i have are Holidays,parental leave,unpaid holidays, day off. Thank you0

As you are on zero hours, you accrue holiday in respect of every hour worked.
Assuming that an employee with contracted hours receives the statutory minimum of 5.6 weeks holiday, you would have accrued, as holiday to be taken, 12.07% of the hours worked so far in the holiday year. (If a full-time person would receive more than the statutory entitlement, then that percentage would increase but 12.07% can be taken as a minimum.)
Not seeing the actual form or any notes about its completion, I can't be sure but I think I would want to complete the form to request (paid) holiday on the days I would normally expect to work and use the "days off" part to record when I was not available to be called in to work. But do check what they mean by a "day off" in case they mean a day off for a reason like a hospital appointment or compassionate leave to attend a funeral.
Do you need to say how many hours a day is for paid holiday? As your normally work between 4 and 6 hours, you may want to suggest 5 hours.0 -
As you're on zero hours, does that mean that they could, theoretically, offer you work on the days you don't normally work?
If that's the case, I'd book the 8 days' leave, but make it clear that you're not available for work on the days you don't normally work. Without seeing the form I don't know how you'd do this, but if it's a case of ticking a box or writing something on each day, I'd maybe put a big fat X through each day you don't normally work.Signature removed for peace of mind0
