I would never divulge my previous sick record in any application or at interview. In fact, in most cases it is illegal for them to ask you.
As others have said, many employers now require you to fill in a health questionnaire after you have been offered the job. In most cases they can't then withdraw the job offer on the basis of this and would be guilty of discrimination if they did.3 stone down, 3 more to go0 -

It's on every application form I have ever seen, how many days sickness in the last 12/24 months. I'm very certain it's not illegal as it's very common.Tomorrow is the most important thing in life0 -

Dear boss
As you know, I have been signed off for the past six months and have now decided that I will not be returning. I am therefore giving you a month's notice, and as I am still signed off and do not expect to return to work I will arrange a time with you to collect my personal effects and return my keys.
I believe that by the end of my notice period I will have accrued 10 days' leave which I have not been able to take, and would like this to be paid with my final salary.

Just because it's common doesn't mean it's not illegal. I have looked into this but don't have time to post a link right now.
I have applied to 4 different organisations for jobs. Only one had questions about sickness absence on application forms. They don't any longer as someone may have pointed out that they were breaking the law.
There are certain exceptions, where it would be impossible or extremely dangerous to do certain jobs with certain medical conditions.
I have also seen several job ads posted recently that were almost certainly breaking the law regarding age discrimination, by posting that they wanted certain age groups.3 stone down, 3 more to go0 -
