Surprise delivery! (Help?)

I have just received a surprise delivery of a Platronic gaming headset:j
I had no idea I had won and it got taken to the wrong address (Luckily got redirected here even though the delivery slip stated another address but my name)
- Scary thought that if it hadn't have been redirected I wouldn't have even known it was missing because I had no knowledge of winning.
There is a slip that says Purchase order for Heaven Media % winners but no other indication of where the prize came from.
I enter for most gaming things but can't find anything. It's possible I could have entered from another site but I am on a different laptop so not much history on this one.
