Formatting nightmare of 6.28 form.

I'm having a nightmare filling in the forms, section 2 about self employment seems to be 2 tables on top of each other and try as I may, I can't sort it out. I use open office writer, I haven't got word., but have never experienced this before. I've tried printing that section out so I can handwrite it, but it prints out as 2 forms on top of each other.
I've tried searching for a blank pdf but no luck. Has anyone else experienced this and what did you do?

Where do I put how much I owe on credit cards? The guidance note for Section 4 doesn't include credit cards and section 5 just asks for the details of the cards, not the amounts.
I spent so long on the statement earlier in the week etc that I'm now a nervous wreck over the nuts and bolts bit which should be easy!!!0 -
They do go in s4.
