Worried about health questionnaire

I've got an interview next week and along with the interview letter came a really in depth health questionnare. I'm a bit worried as I've realised my health record over the last year looks awful! I was signed off with depression for three months last year and found out I have an underactive thyroid gland which has led to lots of little niggles like weight gain (I'm now in the obese category), tiredness, repeated infections...
Basically I'm worried that these probs will stop me getting a job even though I feel fine now - it just all looks so bad; obese, loads of time off and history of depression. It's a job in a NHS call centre. I mentioned on the application form that I had a lot of time off last year and that obviously hasn't put them off but I'd appreciate any info from people who have had similar questionnaires or health checks and whether they came across any problems.
Any advice on handling tricky questions? I left my job in January after only being back for a few weeks after a period of depression which also doesn't look good though the reason for leaving wasn't because I was depressed. I've kept myself busy with an Open Uni course and I'm keen to get back to work but I'm really worried that I won't be given the chance.

This is a toughy,personally I`d tell the truth and at the bottom write that you are recieving the right medical treatment and/or recieving other forms of treatment and do not expect this will interfior(sp) with your future employment.
But the main question I`d be asking myself is what is causing all my depression.Will this new job break me free from it and give me a new lease of life?

I agree that you need to be honest, but stress that things are improving now you have the right diagnosis and treatment.
Not all employers ask for these in-depth questionnaires. I remember getting one for a Civil Service job - "because we have to make sure that the govt gets good value for money from you" is a paraphrase of their reasoning for this - and it put me off so much I never filled it in. Carried on job-hunting and got one with no difficulty.Signature removed for peace of mind0
