How am I supposed to remember when I was sick 2 years ago?

I have to fill out a paper and I have to give dates and reasons for me being absent from work, which they (Humberside Police) say they will verify.
I know I was sick 5-6 times. Food poisoning and headache (I think), but I have no idea about the dates. Do I just call my employer? Will they even remember?

Contact Humberside police & ask for clarification. There are probably worried about stuff like "I was repeatedly off sick with the same complaint which has not got better". A "I had a few days off with food poisoning/head ache/hangover:-) etc" is probably not an issue0

They want to know (assuming Humberside Police are your potential new employer) to see
- if you have any repeated absences which might bode badly for future sickness
- if you have lots of uncertified days off
- (possibly) if your record of sickness is the same as your employer's.
Your employer should have a record. If they have not recorded it all, I would just tell Humberside about the records your employer has!Ex board guide. Signature now changed (if you know, you know).0
