help! with health form

I have an interview for an admin job with the NHS and have to fill out a occupational health form . Will have to go to GPs for vaccination info. does neone know if they charge for this? and am confused about whether i need to provide evidence for rubella/varecella immunity or is it juct for doctors etc? thanks for help

    My doctors charge £19 for a health/vaccination certificate. Not sure if you'd need the evidence for jabs for an admin job but it's the NHS so it wouldn't come as a shock.

    You will need these for a job in the NHS, (as well as a course of Hep) as usually the violent/aggressive patients do not check your ID badge to ensure you are a nurse or doctor before attacking you!

    Make an appointment to see your practice nurse who will check your records for you. Do not worry if no records can be found relating to childhood immunisations, as this documentation is often found to have gone astray during the transition from paper based to computerised notes. OH dept can do blood tests to check your immunity.

    I have been working as an administrator in an NHS clinic as an agency temp for a few weeks now. No one ever asked about my health: should I be concerned?

    Can only say that whenever I've had to have a medical pre-employment, the employer has always borne the cost. Seems to me that if the NHS wants "evidence" they should pay.

    The questionaire should be a generic one with the expectation that you know what you've been vaccinated against.
