how to apply for mortgage offer extension? how likely is it to get it?

I currently have a mortgage offer with Santander which expires 31st July. We are very close to exchange, just waiting for the last two letters to arrive. Based on the minimum notice period of 5 working days to completion we have to exchange by 23-24.
How should I go about asking for an extention of the offer?
The rate have gone up since then and the product is not on the market anymore plus there is MMR legislation in place. What are the chances I will get an extension of the same offer?

My circumstances are the same or little bit improved - a slight pay rise and a small bonus + credit card debt reduced from 900 to 300.
