Could it be that the real reason is that during your absence, they've had to rejuggle your responsibilities and they like the way the team now works (or maybe have employed someone to replace you and they want to keep them), so the move to another department is not to punish you but what suits the company in light of the adjustments they had to make when you were off for 4 months.

It sounds to me like they are making what would be considered 'reasonable adjustments' to your role to enable you to return to work. As a union rep this is something I often negotiate on for members and usually it's the best way to get someone off sick and back to work.
Relying on your friend in the office's understanding of this conversation they've heard seems to have caused a bit of unnecessary stress for you, your employer sounds like they're being fairly reasonable. If you've told them you don't feel ready to go back to your old role but your fit note is coming to an end the best thing they can do is find you something else to do.

Best to speak to them directly about your concerns.

Yes, remember however they are only actually obliged to make "reasonable adjustments" if your illness amounts to a disability for employment law purposes.

If that is not the case they could just insist you do your normal full job or stay off sick.

The type of illness you indicate can be a disability under certain circumstances but generally isn't.

So it seems they are doing more than the minimum the law requires.
