ESA medical report

Does the medical report for ESA have to be from your GP -or can it be from any health care professional associated with the claimant such as a consultant, social worker, mentor etc?

The medical report is filled in by a GP.
You can provide supporting evidence from consultants, social workers or other professionals with the application; the guidance notes do tell you to send anything in that you have available in order for a decision to be made.0 -
Ah , my GP says there is a charge for report £25 for detailed one , £15 for basic-anyone clarify ?
Also my son is on Child Benefit and part of my tax credits-so i assume ESA claim should start from his 20th birthday ?0
