I agree with everyone else and tell the truth. Burying your head in the sand only makes matters worse.
Mistakes happens. The trick is to learn from them and if possible recommend solutions to help prevent future similar mistakes.

The only people I have worked with who never made mistakes were either the ones that did not do anything or the liars.
