    Its probably an error caused by a gap in her tax records, but ignoring the request won't make it go away. If HMRC ask you to complete a tax return its compulsory, its for them to decide not you.

    Fill it in and return it, if she doesn't owe any tax then they may well write off any fines.

    Get her to phone them. I've successfully got HMRC to withdraw notices to submit an SA return where it's clear that there's no reason for them to have asked for it. Sometimes HMRC make mistakes and it's quite possible that, in this case, someone has "pressed the wrong button" to trigger SA returns being asked for. If she speaks to them, they *may* "press the button" to make it all go away and then the penalties will be cancelled. It's surely got to be worth a try? You have to remember that the whole SA return and penalties regime is automated - by submitting a return, you're effectively accepting the situation and it will be harder to get the penalties reduced. Far better to get the requirement to submit them removed which clears everything.

    Being on PAYE does not necessarily mean that you won't be required to self assess. http://www.hmrc.gov.uk/sa/need-tax-return.htm

    "If you have relatively straightforward tax affairs and already pay tax through PAYE (Pay As You Earn) you probably won't need to complete a tax return."

    "If you don't think you need a tax return
    If HMRC has asked you to complete a tax return and you've checked the information above and think you don't need one, call the Self Assessment Helpline. They will tell you whether you still need to complete a tax return."

    Your wife should have rung HMRC immediately to explain her situation.
