Shared Equity Mortgage - Poor Credit History

I am looking at an shared equity property which is £275000
My partner has been a council tenant for 22 years and is eligible for a grant of between 38 and 60K as a deposit against buying a property, giving a deposit of up to 60k.
The issue is that I have a poor history from credit card debt from a previous relationship and have been in an arrangement with 4 lenders since 2011.
We both work full time with a joint income of around 38k and my partners credit score is good.
what are the chances of us getting a mortgage on say 25% of the property initially and who does them?
