I wouldn't read off a piece of paper, no. They won't expect you to give a 500 word essay but it will help you know a little background information about both the Company and the job you're going for.

Be yourself. As you said, be polite, make eye contact, smile; try to relax. Answer any questions you are asked concisely; try not to babble and ramble on. If there is something you don't understand then ask them to repeat the question. If your mind is blank then say "I'll come back to that question later if I may."

Make sure you are dressed appropriately, and the obvious stuff- make you've had a bath, washed your hair; if you're male then either shave or make sure you're tidy.

Another suggestion is to have a couple of questions prepared to ask them, usually you get asked if you have any questions, if you make sure you have 2 ready that way if they answer 1 in their general chat about the job/company you have another one ready. Usually something to do with progression in the company is a good one, as it implies you are committed to the long haul if they give you the job, and that you are eager.
