CPP confusion

Hi, my wife has received a CPP claim form stating that she may be entitled to compensation. It contains a unique reference & policy number. From the info on this site, that alone suggests that she probably has a claim.
Unfortunately, she doesn't know to which product it relates; between us we have moved house 3 times & got married in the past 18 months and are not able to find any relevant correspondence.
It seems reasonable to request further information in order to complete the claim. Does anyone know if CPP will be able to enlighten her?
