Just a note to say the form on the justice/ gov website has changed-I downloaded and sent off the form, only for it to be returned from the court with a booklet and new form attached!

I found this yesterday too. They told me I must have printed it from another website but I haven't had a chance to check it yet. I was given a new form too.SPC 8 (2015) #485 TOTAL: £334.65

The one on the main thread here is the latest from:
