New form pertaining to job searches

All it is, the weekly form used to record job applications and effortts made to find employment.
As of 26Th Feb there are now a collection of boxes to tick that include such things as 'I have not done voluntary work', 'I was available to start work every day'.
Where I live in south warwickshire these boxes have not existed for the last year that I know of.
I'd be interested to share the experience of others. After all if some thing is not broken why fiddle about with it?

I've seen and produced similar job logs and its nothing really new, they are the statements at the top of the signing coupon that you agree to when you sign.0 -

Forms re totally different in my neck of the woods.
Just a booklet. Each page split in two.
Top half is a big blank box where you write in what action you will be taking in the next fortnight.
Bottom half of page is another blank box where you write in the actions you have taken.0 -
