DMP onto mortgage?

FIRST POST, SO PLEASE BEAR WITH ME!!!:eek::eek:
I spend a lot of my time thinking of ways to best sort my finance situation out and think i need to speak to same minded people.
My situation;
I have had to set up a DMP with stepchange and pay them £160/month to cover credit card, old overdraft etc.
I also pay my wife's debt every month totalling £150/month (minimum payments).
We have a fixed rate mortgage which expires in feb 2016 and is currently at 4.74%. This is santanders varaible rate at present.
i understand the implications of bad credit score due to DMP, but i am on decent salary and wondered if i may be able to put the debt amount onto a new mortgage with santander as an existing customer and save /month now.
Any ideas please????
