Paperwork re Means To Pay

Can anyone point me to further information or possibly online copies of the paperwork issued by CSA requesting Means To Pay details please?
My case has now been passed to the Legal Enforcement team to progress to Liability Order. The lady who called me advised they would be sending a form to the NRP requesting Means To Pay information. She said a similar form will also be sent to me to ask if I can provide any helpful information I may have about the NRPs place of residence, type of employment, income details etc. I have tried to find copies online or further information but am struggling to find anything.

The most common form we issue for method of collection is called an 808a, it does just as it says on the tin, asks the nrp for a method he wants to pay his maintenance, failure to provide one results in a DEO, unless hes self emp.0

This doesn't sound like a CSF 808, more likely something specific to Legal Enforcement.0

Prelude, I think you are right in that it's possibly something specific to legal team.
However, I had a call today to say they had managed to contact the NRP and have made yet another arrangement for payments. Still not got a payment from him but despite him not keeping to promises to pay over the past few months, they have decided to trust him!!
The current arrangement, calculated to include the normal weekly assessment amount and clear the arrears comes to weekly payments of four times the normal assessment amount. This arrangement will be lasting 5 weeks. I am not holding out any hope of getting even one payment!!
