Bankruptcy / name-change issue

Hi folks,
It's been recommended to me that I start a thread here as I'd really appreciate your help on my particulars of declaring myself bankrupt.
I've searched and searched for weeks and as yet can't find the specific answer to my specific situation, so any help would be really useful.
I'm considering bankruptcy, but I've changed my name via Deedpoll several years ago and about half of my debt is under my old name, the other half being under my present name.
How would one go about declaring themselves bankrupt if their debt is split between their old and new names, since I'm no longer allowed to use my old name in any way?
For example, would I declare myself bankrupt under my current name and include my Deepoll in my forms with a written explanation or something, or is there likely to be a problem due to there being debt in my old and new names? In other words is there a set process for handling this sort of situation?
When I changed my name it was purely for personal reasons, not to try and avoid debt, but now that I'm considering bankruptcy I'm confused regarding how to do it since I'll effectively be writing off different debts under two different names.
Does anyone have any experience of this specific issue, and if so would you be so kind as to share it with me as I'm pulling my hair out on this one?

The debts are all yours, no matter what name you used at the time and will be covered by any bankruptcy.
Question 1(b) on the court forms is:
and it is as simple as filling in any previous name there. All covered.1 (b) Insert in full any other name(s) by which the debtor is or has been known

Sometimes I can't see the wood for the trees and it hadn't occurred to me to look at the forms and figure that one out on my own!
