Self Assessment & earnings from foreign share dividends

Hi,
Hopefully someone can confirm what should be done within your self assessment in relation to foreign share dividends.
If you own x number of US based shares and you are paid the gross amount of the dividends less U.S Withholding Tax for the tax year and this figure is less than £300, what do you need to enter into questions 5 & 6 in the income section:
5) Foreign Dividends (Upto £300)
6) Tax taken off foreign dividends
