Bankruptcy discharged -trying to sort out Credit file

LILA Bankruptcy was discharged on 19 Nov 2013, I have gone through Equifax to have a look at how bad!! my credit file is, most of the account are now showing as settled, but there is one that is listed twice it was a Barclaycard credit card it is showing as settled but also comes up again as what assume is a debt collection agency and is showing as defaulted but is the same account can I/should I do anything about this??

How is the original default marked? Does it say in any way that the debt has been assigned or sold?Free/impartial debt advice: National Debtline | StepChange Debt Charity | Find your local CAB

I cant find anything that says anything like that! where would I find it? It says default date 25/05/2013 date settled 25/05/2013 and status as settled and nothing further after the month of June 2013 shows up on the payment history.
