I usually just put name, address and phone no but each to their own I think. Love it! just send prizes through the post so not aware of anyone ever receiving a call.

I have my email set up to give:

name
address
phone
email

as several sites ask for all of those, and it's far easier if it's done automatically than for me to have to input every time.
