Business mileage for community care workers

Does anyone know the rules on what is regarded business miles for care workers who are based at home on the whole?

As i understand it, business miles includes any travel incurred to a non permanent place of work, so would this include travel from home to the first client, and travel from the last client to home? especially if the clients were to vary on each shift worked?

The reason i ask is my wifes employer pays business miles, but not travel to/from home from clients houses, which I am not sure is correct, but I also know its not necessarily the employers duty to manage any claim here
