Paperwork for OR

Hi guys

I have a query regarding the paperwork.

The OR has asked me for

Loan/Finance Agreements
Bank & CC statements for last 12 months
Used and unused chequebooks
Bank or credit cards still held

That's fine - got all organised with no problem but it also has a list I have to fill out with dates and descriptions etc called the schedule of books, papers and records?

Is this the same as above or are they asking for my accounts (I am self employed) as well as what is above.

If you need any more info please let me know

    Think this might be a form that you take with you if you are having a face to face interview as it has to be signed by the OR when documents are received.
