The form was there on 19th/20th and it is not accessible through the menus either. What may have happened is that many comps closed on 20th Feb and their forms were deleted sometime on the 21st. They could have included issue 4 as one of the deletions in error. This has happened before on the odd occasion.
If someone contacts them on puzzlesfeedback@bauer.co.uk to let them know they are usually very good at correcting any problem, although it will probably not be until after the weekend.
