    didn't work in chrome for me either - OK in explorer

    Ha! Strange, opening the original link on my iphone produced a completely whole different page with all the details to enter there clear as day!

    To get it to work in Chrome, you need to click on the shield button, top right of screen, and then click on "Load unsafe script". That brings up the entry form. Sounds a bit dodgy but it works!
