Form 6.28 Help please

Hi all
Wife and I have BR day on 27th. Have completed our forms but am unsure about the first 2 pages of 6.28. How much of this bit needs completing or is this done at the court?
The first bit asks Re - is that my name there?
The next bit is Sworn at - is that the name of the court?
Do I date it now or date of petition?
The 2nd page is much the same. Is this for the court to complete?

Yes, the court staff complete all that gubbins. I struggled for ages with that bit until someone told me, so you're not alone.
