You don't need the survey number. Just fill the ticket out best you can and send it in.
You don't need a screenshot either.

They do like you to include the URL of the last page you had on the survey if you have it, but if you don't have that, then don't worry.

Oh right ok, thanks for that I wish they would ask the criteria questions right at the beginning though. Save a lot of time and effort for everybody.
