The form is to the right of the giveaway page (right of the T&C box).
It worked for me and I got an email confirmation with £20 off too.

I keep getting a "Error establishing a database connection" message.

This offer feels a little suspicious, but I can't see that it is. Though I expect if we get anything at all, we won't consider it to be worth £10.
