Tax relief on mileage for work?

Hi,
I work as a carer and get a fixed amount for mileage per call, rather than being paid per mile. How do I go about claiming tax relief on the rest, as the payment certainly doesn't cover fuel for some of the calls. Do I need to record my mileage for every call?

Keep a record of your total mileage for the tax year.
Multiply the first 10,000 miles by 45p per mile.
Multiply the rest by 25p per mile.
Add these two together, and deduct the total amount your employer's paid to you.
You can claim tax relief (20% if you're a basic rate taxpayer) on this amount.
You would claim it by completing a p87 form from HMRC.0
