Help with Debtors Bankruptcy Petition

Hi i've got to question 4 ii and iii on the form and am now well and truly stuck, the reason being is that in april 2011 my husband and i entered into an IVA but only made payment until sept 2011 it failed in July 2012 and we have not made any further arrangements with any creditors, how do we complete this section ii. Do i cross out any of this section and also section iii do i put the date of the IVA in there, i have managed to fill in all the other forms and was hoping someone may have the answer, thanks in anticipation.
