Help - Old Prudential Policy

I have had a letter from Prudential asking for personal details (name, address, previous addresses, phone numbers and National Insurance number) to confirm if we had an old policy with them.
We are unsure of whether we have any outstanding polices but very reluctant to fill in the form in case it is a scam.

Prudential does send out this sort of thing - it might well be for a contracted out pension.
If you're unsure, send it to Prudential's registered address:
