According to the masthead of today's Telegraph this is redeemable at Whittard shops only, but is valid all this week (though presumably they may run of out of promotional stock if there are too many takers).
So you need the token from the Telegraph (and can get that free if you buy in Waitrose, spend £5 and have a Waitrose card)
And you need a Whittard shop. Which are not all that common - locations are given here http://www.whittard.co.uk/locations

Got the praline hot chocolate today. Didn't realise it was white chocolate, was a bit disappointed. You also get very little in the tin, there's no way i'd pay £5 for it.0
