Thanks for coming to the survey. We have put together a few questions which should only take a few minutes to answer.
All entries will be entered into a prize draw to win one of the prizes up for grabs!

First prize is an iPad mini (16GB wi-fi only model)
Second prize is an original cover art sketch
Third prize is one of ten original Roger Mellie artworks.

On completing and submitting this survey, you will automatically be entered into a draw to win a main prize of an iPad mini (16GB wi-fi only model), original cover art sketch or one of ten original Roger Mellie artworks. No correspondence will be entered into and the winners will be notified by post or email within 28 days of the closing date. The competition is not open to employees of Dennis Publishing or participating companies. No cash alternative will be offered. The prize(s) described are available at the date of publication. Events may occur that render the promotion or the awarding of the prize impossible due to reasons beyond Dennis Publishing's (publisher of Viz Comic) control, which may at its discretion vary or amend the promotion and the reader agrees that no liability shall be attached to Dennis Publishing as a result thereof. Having now done it you can click through some pages with no answer - took me about 5 minutes (but I filled things in).

It does state in the survey terms and conditions that they will share your information with third parties so beware of this before you submit!
