    You can not tell a pervious employer what you want them to write word for word

    If you don't like the reference then don't offer them to the new employer.

    You cannot pick and choose your reference unless you are on very good terms with the old employer.

    Also, if say you get only the certain amounts and the new employer asks for more and they say, no sorry this is all we are giving.....that would make me suspicious of what went wrong.

    No for all answers unless you have a compromise agreement in place which it doesn't sound like you do.

    No you have no right to dictate to the employer that they should not fill out a questionnaire unless you have an agreement already in place or they do not do that as per company policy.

    I know my old company only give a standard reference and will not fill out a form.
