    If it is the neon post-it notes you can use the form from the 3M website:
    http://www5.3m.com/intl/uk/apprenticeuk/

    If it is the yellow post-it notes, the offer is exclusive to PC World and you need a pcworld claim form. Either from the website or in store. If you cannot find get hold of any I can mail you some as I have spares.

    I dont know if it is my printer at fault but i cannot get the form to print off from the 3m website, and i still can't get the one from pc world to load up. If you still have spare copies i would be very grateful if you would post me two.

    To view the form it you will need an "Adobe Reader":

    I can post a few original claim forms to a few users without printers if required...
