I had the same survey went white at end how do you put in a ticket bout this no idea what survey number is....

You can submit a ticket by going via the Help Center (on the homepage, scroll right down to the bottom).
There's a link on the Help Center page, something like "is your question not answered here" and you can contact SB there, about surveys and stuff, where it says "Submit Help request".

Not sure if it will do any good, but worth a shot I guess.

The other day on the facebook page there was a discussion about this issue, the white screen has been going on with multiple surveys for quite some time now. SB [in the main] are not crediting people for these, claiming it was the survey company's fault.

Apparently some people did made requests to the survey company direct, but got a response to the effect that they will refuse to provide evidence of screenout / explain what they did with your personal data, even though (presumably) this violates their t&c of the data protection act.
Not sure how long this discussion would have survived on the FB pages though.
