How to apply for a A Jobcentre Plus Travel Discount Card

Hello everybody,
I have been on JSA for more then 3 months and as far as I could understand from the website of tfl
http://www.tfl.gov.uk/tickets/14423.aspx
if you're 25 or over and have been unemployed for three to 12 months
you ca apply for such a card
Now, I would like to apply for it in a written form.
Is there anybody here who applied for this discount card? If yes, what could I write in my application?
Any suggestions?

What I ment was:
since I would like to make the application in a written form, i was wondering what I could write in.
I was thinkg something like:
Dear Mr X
I would hereby like to apply for the Jobcentre Plus Travel Discount Card.
According to the information extracted from the TFL (Transport for London) website the eligibility for this application starts when being unemployed and on Jobseeker's Allowance for a period of 3 months.
This makes me eligible, therefore I would really appreciate it if you could be so kind as to approve my application.

Book an appointment with your jobcentre advisor specifically about this.
Take two passport sized photos with you.
Sorry, but my understanding is that you need to apply in person.

I have prepared an application in written form.
I hope my agent will approve it.0 -
So, for those who live in London and need an Oystercard Discount Card.
You have to be on JSA at least for 3 months and - above all - you have to prove that you have been travelling to the city for job interview or like in my case for marketing research for I have been preparing my business plan.
Because your agent has to justify why are did he/she approve your application.
My agent gave me a new appointment for next week to forward my application :j0 -
I had one of these earlier in the year. I just asked at my jobcentre.
TBH I thought that the scheme ended ages ago, well it did in the North West.Bedroom Tax / Spare room subsidy / Housing Benefit Reduction - It's the same thing, get over it.0
