Can I send a sick note (for 2 weeks) and my resignation note (last day at work will be on the the last day of my sick note) in the same letter to my employer ?

Yes, if that's the notice period in your contract.0

Why are you resigning while off sick?0

You could but you may find any discretionary sick pay you may get, could be stopped.
If you just get SSP or contracted SP, go for it if that's all the notice you need to give.0 -
