very old debt very new default

hello , I am hoping for some advice I tried to balance transfer my credit card today and was very surprised to be refused ,I checked my credit file and found that a company called Mackenzie hall have placed a default on my file for a unacknowledged debt from 2004 , I am now very confused about how they can still do this with a debt this old and they have destroyed my credit rating , I remember the incident in 2004 it involved a dodgy neighbour setting up a catalogue in my name ,ordering and intercepting the delivery as she knew we were at work , this was all resolved by myself and the catalogue company at the time , now this I have no idea where my former neighbour is , the catalogue company are bust , I know I don't owe this ?? how on earth do I get this sorted , and the default removed asap ?????????????????????

Well, each case is a little different so there is no one size fits all template.
However have a read of the ICO guidance.

A new default for such an old debt is completely inaccurate, and as such recording one is a breach of several parts of the Data Protection Act and principles.
Also deliberately recording false information on your credit file can be regarded as defamation.
