Debt Agency Collecting same Debt Twice?

I hoping on some information on my next steps on how to find out if I am being asked to repay the same debt twice.
Background story
My finances were in a mess and over the last few years I have entered into repayments and done everything recommended to increase my credit rating. I settled/paid all my defaults and used credit cards to help improve my rating.
I got a personal loan of my bank 18months ago and paid on time every month without missing a payment. I assumed by doing so that I would be able to obtain another small personal loan to repay again and further help my credit score.
Fast forward and I was rejected for said loan due to an unsettled default. I have obtained my credit files and a new default was recently placed on Experian. I also received a letter at home from the debt agency stating the details of the default and amount.
However, I am 90% sure that this debt has already been settled with another agency. On my credit file it does not give the original debtors information so cannot be certain.
What is my first step in trying to sort this out?
This default is now on my file and my file was going to be completely clean of defaults this year. I feel this has set me back terribly in obtaining a mortgage with my partner.

Have you checked your credit file to see what's on there?
As said, prove it letter or if you know what debt is and the default date send them a written complaint and tell them to correct the date or you will report to OFT, ICO and escalate to FOS.:beer:0
