Hi Queenijenni. I used this template and got a solicitor to witness it. He said the wording was fine and I think I paid him a fiver.

It basically said that I renounced the use of my short name and will only use my long name from now on. It wasn't really what I wanted to do but it was enough to get all the paper work processed. I do still use my short name informally.

You will pay £5 for a solicitor to witness the Stat Dec.
You can also do it for free at a court (Court staff are officers of the court authorised to adminsiter oaths, which includes Stat. Decs)
If you need certified copies of the stat dec once it has been made Which you may, if you need to provide it to more than one organisation) then you may have to pay for these separately. You don't want to renounce either of your names, you either want to declare that the certificate awarded in the name of x is in fact referring to the person known as y. Or more generally that the person known as x is also known sometimes as y.
A solicitor will draw up the required document and will charge a very small fee.0
