You need these forms:
Debtor’s Bankruptcy Petition
http://www.insolvency.gov.uk/pdfs/forms/form6-27.pdf
It must be submitted to the court with this form:
'Statement of Affairs (Debtor's Petition)'
http://www.insolvency.gov.uk/pdfs/forms/6-28.pdf
The guidance notes for filling in 6-28 are at:
http://www.insolvency.gov.uk/pdfs/forms/debtorsguidance.pdf
You would be advised to also read this factsheet called:
'How to petition for your own bankruptcy' at
http://www.insolvency.gov.uk/guidanceleaflets/dealingwithdebt/howtopetition.htm
Hope that helps

Just to add...
You need to book a date to go bankrupt. You can't just turn up and expect to be dealt with. When you go to your local County Court to book a date, they'll give you all the forms there.
In my area County Court, it's currently a 3 month waiting list. Don't hang about.0 -
